Elder Care Clinician Program
Compassionate mental health support, delivered at home
SVCOA’s Elder Care Clinician Program offers in-home mental and emotional health services to older Vermonters who are primarily homebound or unable to access traditional, office-based care.
This program is a partnership between Southwestern Vermont Council on Aging and Rutland Mental Health Services, focused on meeting people where they are—both physically and emotionally.
Our licensed elder care clinicians provide personalized care in the comfort and privacy of the home. Services may include:
- Mental health counseling
- Emotional support
- Stress and coping strategies
- Help managing depression, anxiety, and isolation
- Referrals to additional services if needed
- Support for caregivers and families
Each care plan is tailored to the individual's needs, with a focus on comfort, trust, and access.
